เอกสารที่ต้องยื่นพร้อมคำขอรับใบอนุญาต
1.สำเนาทะเบียนบ้าน หรือสำเนาบัตรประชาชน
- กรณีผู้ขอรับใบอนุญาตเป็นบุคคลธรรมดา สำเนาทะเบียนบ้าน หรือสำเนาบัตรประชาชน ของผู้ขอ ผู้รับมอบอำนาจ และหรือผู้ให้ความยินยอม
- กรณีผู้ขอรับใบอนุญาตเป็นนิติบุคคล สำเนาทะเบียนบ้านหรือสำเนาบัตรประชาชน ของกรรมการผู้มีอำนาจลงลายมือชื่อ ผู้รับมอบอำนาจและหรือผู้ให้ความยินยอม
2. สำเนาหลักฐานการมีกรรมสิทธิ์หรือสิทธิครอบครอง เช่น โฉนดที่ดิน น.ส.3 หนังสือยินยอมให้ใช้ที่ดิน หนังสือรับรองสิทธิที่จะเจาะน้ำบาดาลในที่ดิน หรือลงชื่อรับรองว่าเป็นผู้มีสิทธิในที่ดินในคำขอรับ ใบอนุญาตเจาะน้ำบาดาล
3. แผนที่สังเขปแสดงเส้นทางและสถานที่ที่จะเจาะน้ำบาดาล
4. สำเนาใบอนุญาตประกอบกิจการโรงงาน หรือใบอนุญาตขยายโรงงาน (กรณีเป็นโรงงานอุตสาหกรรม)
5. สำเนาหนังสือรับรองช่างเจาะและหนังสือรับรองนักธรณีวิทยา หรือวิศวกร (กรณีระบุผู้รับเหมาเจาะ)
6. แบบแปลนจัดสรร (กรณีเป็นหมู่บ้านจัดสรรหรือที่ดินจัดสรร)
- กรณีผู้ขอรับใบอนุญาตเป็นบุคคลธรรมดา หนังสือมอบอำนาจปิดอากรแสตมป์ 30 บาท (กรณีไม่ได้มายื่นคำขอด้วยตนเอง)
- กรณีผู้ขอรับใบอนุญาตเป็นนิติบุคคล
1. หนังสือมอบอำนาจปิดอากรแสตมป์ 30 บาท (กรณีกรรมการผู้จัดการไม่ได้มายื่นคำขอ ด้วยต้นเอง)
2. สำเนาหนังสือรับรองการจดทะเบียนนิติบุคคล แสดงรายนามผู้มีอำนาจลงนาม
2.หน้าที่ของผู้รับอนุญาตเจาะน้ำบาดาลฃฃ
2.1 ก่อนที่จะเริ่มเจาะน้ำบาดาลจะต้องนำพนักงานเจ้าหน้าที่ไปตรวจสอบการเตรียมการเพื่อเจาะ น้ำบาดาลเสียก่อนจึงเจาะน้ำบาดาลได้

4. Copy of factory business license or factory expansion license (In the case of an industrial factory)

5. Copy of certificate of drill technician and certificate of geologist or engineer (in case of specifying drilling contractor)

6. Allocation plan (In the case of an allotted village or allocated land)

- In case the applicant is a natural person Power of attorney affixing a stamp duty of 30 baht (in case of not submitting the application in person)

- In case the applicant is a juristic person

1. Power of attorney affixing a stamp duty of 30 baht (in case the managing director does not submit the application by himself)

2. Copy of juristic person registration certificate Show the name of the authorized signatory

1. 加盖印花税 30 泰铢的授权书（万一董事总经理不亲自提交申请）。

2.法人登记证复印件 显示授权签字人姓名
